DESCRIPTION OF COVERAGE Accidental Death and Dismemberment Provides a benefit for accidental loss of life, sight, speech, hearing or severance of a hand or foot. Such loss must be total and irrecoverable. * Accidental Death, Dismemberment and Plegia In addition to Accidental Death and Dismemberment, a benefit is provided for the permanent paralysis of two limbs without severance, due to a covered accident. The paralysis must result in complete and irrecoverable loss of use of the limbs.* The above benefits are payable to the employee; except benefits for loss of life are payable to the beneficiary named by the employee. In most states, the employer may not be named beneficiary under this group insurance plan. Benefit payments may be paid in a lump sum, or payment in installments may be arranged * The accident must occur while insured. The loss must result directly from the injury and from no other causes and must be sustained within 365 days after the accident. ELIGIBLE PERSONS The Voluntary Group Accidental Death and Dismemberment plan is designed for an employer/employee group with a minimum of 10 eligible persons. TYPES OF PLANS AVAILABLE Employee-Only Plan Under this plan, only the employee is insured -- not his/ her spouse or any dependents. Employee & Family Plan This plan insures the employee, any spouse under age 70 and any unmarried, dependant children under age 19 (25 for full time students). The Principal Sum (Maximum Benefit) for each dependant depends upon the family composition at the time of the loss. If spouse and eligible child(ren)... The spouse is insured for an amount equal to 40% of the employee's Principal Sum and each child is insured for an amount equal to 10% of the employee's Principal Sum. If spouse but no eligible child(ren)... The spouse is insured for an amount equal to 50% of the employee's Principal Sum If eligible child(ren) but no spouse... Each eligible child will be insured for an amount equal to 15% of the employee's Principal Sum. ADDITIONAL COVERAGE OPTIONS Any of the following coverage options can be added to the base coverage in order to customize your protection. Seat Belt If the insured sustains loss of life or dismemberment due to a covered accident while operating or riding as a passenger in an automobile and wearing a properly fastened seat belt, an additional benefit will be paid for dismemberment, or to the beneficiary for death. Monthly In-Hospital Indemnity If the insured is confined in a hospital within 90 days due to a covered accidental injury, an additional benefit will be paid during the period of confinement. The hospital confinement must be medically necessary to diagnose or treat the injury.

Child Day Care
If either spouse dies due to a covered accident while enrolled in the Employee & Family Plan, an additional benefit will be paid to the surviving spouse for each dependent child under age 13 who attends a licensed day care center. This benefit is payable for up to four consecutive years after the covered accident which caused the spouse death. Common Disaster If an employee and spouse both die due to the same covered accident while enrolled in the Employee & Family Plan, the spouse benefit will be increased to an amount equal to the employee benefit, subject to a combined maximum benefit of $500,000. Education If either spouse dies due to a covered accident while enrolled in the Employee & Family Plan, an additional benefit will be paid for the education of each of his/her eligible dependant children. Each child must enroll as a full-time student in an accredited post-secondary school prior to age 25. This benefits payable for up to four years. Spouse Critical Period If either spouse dies due to a covered accidental injury while enrolled in the Employee & Family Plan, an additional benefit will be paid to the surviving spouse. The spouse must enroll as a full time student in an accredited school within one year after the employee's death. This benefit is payable for up to one year. PRINCIPAL SUM & BENEFIT LIMITS Accidental Death Employers may elect a Principal Sum (Maximum Benefit) limit per person ranging from $25,000 to $1,000,000 (higher limits possible). The benefit limit may be the same for all employees or may differ by class. The Principal Sum is payable if an employee sustains an injury due to a covered accident and that injury directly results in death within 365 days of the accident. Accidental Dismemberment and Plegia The Principal Sum, or a stated percentage, is also payable when such an injury directly results in the permanent total paralysis or the loss of limbs, sights, speech or hearing. The loss must result directly from the injury and from no other causes and must be sustained within 365 days after the accident BENEFIT EXCLUSIONS & LIMITATIONS Benefits are not paid for any loss caused by or resulting from:
